Mechanical Quality Stand-Down Reminder

A Quality stand-down has been scheduled for tomorrow October 23, 2025, on all Plan Group Mechanical project sites, following a near-miss incident during pressure testing of a high-pressure mechanical piping system.

On July 31, during hydrostatic pressure testing of a high-pressure chilled water system at 141 Bay Street, Toronto, a 10” Victaulic pipe coupling disconnected at a test pressure of 600 psi.

The test area had been closed off in advance, a JHA and PSI were conducted, and the TSSA inspector was onsite to witness the test. Plan Group personnel were strategically positioned in anticipation of a failure. Thanks to this planning, no injuries occurred.

Incident Response:

Isolation valves were closed to stop the leak, and cleanup efforts began immediately.
A full investigation was initiated, and the disconnected joint was repaired.
Every coupling in the system was inspected.
The system was successfully re-tested prior to final TSSA approval.
Victaulic was engaged onsite to review proper installation protocols with crews.

The investigation determined the coupling had not been fully seated in the pipe groove. Three of the four internal tabs showed compression; one remained intact, indicating improper installation.

Mandatory Corrective Actions:

As part of this stand-down, all Mechanical Project Teams must review and implement the following:
Develop and implement an enhanced installation procedure and checklist for Victaulic couplings (107V and AGS).
Initiate a stand-down across all projects to highlight the issue and communicate the required procedure.
Deliver enhanced training for Project Management, Supervisors, and workers involved in coupling installation.
Confirm workers have the correct tools in proper working condition.
Apply a visual paint mark to each new coupling to confirm qualified installation.
Supervisors to complete checklist, Coupling Map, and photo documentation confirming inspection.
Engage the Quality Department for field inspection reviews upon area completion.
